*Each camper must have a Facilities Usage/Release & Waiver Liability Form submitted before participation! If Camper was registered for Camp online, the Waiver was completed electronically at that time and it is not necessary to turn in a hardcopy at check-in. If Camper registered any other way, the Waiver must be printed, completed, and turned in at check-in the day Camp begins. Without a Waiver, a Camper will not participate. It's a requirement for Bob Hoffman Basketball Camp!
